Print the application, fill it out, put it in a stamped envelope, and mail it to your county Board of Elections
Check here in about 10 days to make sure it went through

County level, including Court of Appeals and Court of Common Pleas Justices
City, village, and township level, including tax levies and bonds that fund local schools and public safety departments
